We accept new patients and dental referrals. Our office is open Monday through Thursday and always available for emergencies. Emergencies are seen immediately, and time is set aside daily for these visits. Please call (574) 288-4400 for and appointment.
Every attempt is made to keep our fees as reasonable as possible. These fees are comparable with most general dentists' fees, and in many cases, below the national average.
We will submit dental claims to your insurance companies; co-payments are due at the time of service. Any fees not covered by the dental insurance are due by the responsible party within 30 days. Payment for services rendered is due in full within 30 days.
